Questions to Ask
• What accreditations do you have?
• What state licensure do you have?
• How many brain-injured individuals do you serve annually?
• Ask for a copy of their program outcome results for the past year including a
summary of satisfaction surveys.
• Ask about the ownership and management of the company. What is their background and what is their level of involvement in day-to-day
operations?
• Ask to see sample client schedules.
• Who is the medical director? What does he/she do?
• What is the structure of the program? Is there a consistent team? Who are they?
• Who is my case manager?
• Who is in charge of the team?
• How many hours of therapy per day can I expect? What happens the rest of the day? What percentage does a licensed professional provide?
• How does the team work together? How do they communicate? How often will I be “staffed”? Is my family invited? Do I participate?
• How often are reports produced? Will my family automatically get a copy? Are they reviewed with me?
• What kind of regular communication can my family expect from the case
manager?
• What level and kind of communication will the program have with the
funding source?
• How is transportation handled? What happens if we have to cancel?
• What kind and how often is follow-up conducted?
• How is a discharge plan formulated?
• How many hours per week, on average, are services provided emphasizing
community reintegration?
• What about vocational services? How are they provided and by whom? Can I expect trial placement while in the program?
• What family education programs and support groups are in place?
• What kind of support groups are available for clients?
• Is there a client/patient advocate in place?
• How do client complaints get handled? Who do I complain to?
• Find out policy on deductibles, co-pays, billing of non-direct costs (equipment,
orthotics, etc.)
